{"id":3797,"date":"2024-06-18T14:12:45","date_gmt":"2024-06-18T12:12:45","guid":{"rendered":"https:\/\/ifevet.com\/uk\/?p=3797"},"modified":"2025-01-09T09:16:06","modified_gmt":"2025-01-09T09:16:06","slug":"systemic-ischaemia-reperfusion-syndrome","status":"publish","type":"post","link":"https:\/\/ifevet.com\/uk\/systemic-ischaemia-reperfusion-syndrome\/","title":{"rendered":"Systemic Ischaemia-Reperfusion Syndrome"},"content":{"rendered":"<p><strong>Definition<\/strong>: Sepsis-like clinical presentation as patients present with systemic inflammation (SIRS), coagulation activation (DIC) and endothelial disruption.<\/p>\n<p><strong>Clinical signs:<\/strong><\/p>\n<ol style=\"list-style-type: lower-alpha;\">\n<li>tachycardia\/bradycardia<\/li>\n<li>tachypnoea<\/li>\n<li>fever\/hypothermia<\/li>\n<li>hypoperfusion<\/li>\n<li>multi-organ failure<\/li>\n<li>bleeding, oedema, etc.<\/li>\n<\/ol>\n<p><strong>Treatment:<\/strong><\/p>\n<ol style=\"list-style-type: lower-alpha;\">\n<li>Haemodynamic optimisation: use of early goal-directed therapy. Our goals will be a central venous pressure of 0-10 cm H2O, MAP 80-120 mmHg, ScvO2 &gt; 70% and lactate &lt; 2.5 mmol\/L.<\/li>\n<li>Glycaemic control: Hyperglycaemia worsens ischaemic brain damage. Regular insulin is recommended to keep glucose below 180 mg\/dL.<\/li>\n<li>Adrenal dysfunction: Critical illness-related corticosteroid insufficiency (CIRCI) has been described in people after PCP.
